Grant Request for Cloud County Children’s Trust Due on June 9

The annual meeting of the Advisory Committee of the Cloud County Children’s Trust is tentatively scheduled for June 21, 2023 at The Citizens National Bank in Concordia. The Cloud County Children’s Trust has funds available to help children ages 12 years and younger with special medical needs. Children with the greatest financial need will receive priority.  Funds may be applied for by individuals with children in need of services such as: therapies including speech, physical and occupational, special medical equipment, transportation to and from medical treatment facilities, purchase of eyeglasses, hearing aids and payment of certain medical bills.

Organizations working with children ages 12 years and younger may also apply for funding.  The following requirements must be met when applying for a grant:

⦁    Requests will only be accepted for children who were born in Cloud County, Ks, children whose mothers were born in said county, children whose parents now reside in said county, or children who now reside in said county.

⦁    Individuals shall apply by completing an application for request available through The Citizens National Bank, Wealth Management. THERE IS NO DEADLINE FOR APPLICATIONS FROM INDIVIDUALS.

⦁    Organizations or professionals may apply by submitting typed letters of request which set forth the following:

⦁    Description of Request
⦁    Amount of Request
⦁    Specific Reason for the Request
⦁    Other sources of funding for the request

⦁    Every organization submitting a request must submit a copy of its 501 (c) (3) tax-exempt status letter issued by the Internal Revenue Service. Public schools, municipalities and counties are not required to have 501 (c) (3) letters. The request by public schools (Superintendent), municipalities (Mayor) or counties (Commissioners) must be signed by the chief official of the governing body. A request signed only by an employee or department head is not sufficient.

⦁    All organizations receiving funds from the Cloud County Children’s Trust are required to submit documentation of how the grant funds were spent as soon as possible. This documentation should be in the form of a copy of the invoice for the goods or services purchased and must be received by The Citizens National Bank, Wealth Management before another request for assistance from the Trust will be considered.

Requests should be submitted to The Citizens National Bank, Wealth Management, Attn: Amy DeGraff, P.O. Box 409, Concordia, Kansas 66901-0409. The requests must be received from organizations by 5pm, Friday, June 9, 2023.  No late applications will be accepted. Individuals may submit requests at any time.
